A 68 year old man weighing 75kg is admitted with urosepsis. After discussing his urine cultures with microbiology and your senior, you are asked to start gentamicin IV. His bloods are back and his creatinine is 158 micromol/L.


1) Calculate this gentleman’s creatinine clearance using the Cockcroft and Gault equation.

2) Use the BNF to determine whether or not nitrofurantoin is a suitable alternative antibiotic if sensitivities suggested it (assume CrCl equivalent to eGFR in this case)

1) 42 ml/min
2) No, best avoided if eGFR <45ml/min and not suitable for more complication UTI or sepsis
