In an Audit Office report in 1994 called A Prescription for Improvement (Audit Office, 1994) it was suggested that some prescriptions were written to treat new symptoms which could be adverse drug reactions to a previous drug given to the patient. While treating troublesome symptoms is important as we shall see later, ADRs should form part of your differential diagnosis. It may be that there is no choice but to treat these new symptoms but of course you may be able to stop the first drug or at least change it to one that is less likely to cause the problem. The worst adverse drug reactions can lead to anaphylaxis and death
