What should be asked?

This depends on the information you require. If the purpose of the questionnaire/interview is to obtain feedback about the usability of e-learning, the practical template (see link) could be used as a guide for questions. Questions should be specific and objective; individuals may have to justify their answers for feedback to be useful and ultimately enhance usability of e-learning.
