ALPS Annotated Assessment Tool

The following is an example of the ALPS Gaining consent from a service user or carer assessment as if it had been completed by a medical student and then commented upon by their tutor. As the ALPS assessment tools are not levelled and can be completed by any health or social care student there can be no definitive answers but rather the annotated example aims to suggest the sorts of points students should be thinking about when answering each question. The annotated tool has been divided into the 5 separate assessment areas.
