  • Found to be hypertensive on a ‘well man’ check-up
  • On no medications
  • BMI 28
  • Smoker - 20 pack years

BP 170/100 – genuine (if not consider ambulatory BP monitoring)

Yes- stage 2 hypertension (>160/100 - see NICE guidance)

Worried about stroke and heart attack risk

Worried about side effects

What treatment should I use?

British Hypertension Society guidelines/NICE
Step 1:
Patient < 55 years old = ACE Inhibitor
Patient > 55 years old/black = Calcium Channel Blocker
Alongside lifestyle advice- exercise, smoking, diet

What are the patient considerations?

Allergy
Comorbidities
Drug interactions
- None
Practicalities
- Few side-effects
- Once daily dosing
- Will need Urea and electrolytes checking:
Before
2 weeks later after starting
After each dosage titration
